The McGill International String Quartet Academy (MISQA) is happy to welcome you to its next edition edition.

We would like to remind you that MISQA does not provide health insurance to its participants. Obtaining appropriate travel documents and insurance is the sole responsibility of each participant. MISQA accepts no liability of any kind whatsoever for any personal illness or injury sustained by the participant or for loss or damage to a participant’s belongings, including instrument, during transit or while at the academy.

If case of emergency, we ask that each member of the quartet provide us with the name and contact info of the person that should be reached by filling the following form.

I attest that I have proper health and liability insurance for the duration of the Academy. I hereby releases from liability and hold harmless MISQA and its employees, representatives and agents (responsible for arranging travel and overnight housing), for any and all liability for personal illness or injuries (including death), property loss or damages (including instruments) resulting from activities, travel, housing, and accommodation for MISQA events. The undersigned agrees to abide by all the rules and the regulations promulgated by MISQA.

As a member of the String Quartet mentioned above, I will participate in MISQA 2023.

I understand that the McGill International String Quartet Academy (MISQA) does not provide health insurance to its participants and that it is entirely up me to have the proper insurance for my visit to Canada for the duration of the Academy.
